Start Date: November 10 Contract Length: 13 weeks Hours: 40 hours
per week (5x8s) Schedule: Day shift, rotating weekends (currently
every 4th weekend) Position Overview: We’re seeking an experienced
Home Health Registered Nurse for a 13-week assignment in the Palm
Desert, CA area. The ideal candidate is flexible, organized, and
compassionate, with a strong background in delivering high-quality
patient care in a home-based setting. Responsibilities: - Provide
comprehensive nursing care to patients in their homes within a
20–25-mile radius. - Conduct patient assessments, develop care
plans, and evaluate outcomes. - Coordinate with multidisciplinary
teams to ensure continuity of care. - Complete documentation using
HCHB and OASIS systems (training available if needed). - Rotate
weekends as scheduled (every 4th weekend). Requirements: - Active
California RN license - BLS certification (AHA) - Minimum 2 years
of nursing experience - 2 years of Home Health experience required
- Experience with OASIS and HCHB preferred (will train if needed) -
Reliable transportation (mileage reimbursed at $0.70/mile)
Additional Details: - No on-call requirement - All RTO must be
approved in advance and submitted with application - Extension
opportunities may be available - Scrubs: Any color - EMR: HCHB /
OASIS - Parking: Provided at no cost Setting: Home Health
(community-based, travel radius approximately 20–25 miles) About
